Care Mental Health Worker

Provides
mental health assessment, short term intervention for those experiencing mild to
moderate problems and/or signposting to appropriate agencies. Role includes
liaison between Primary Care, specialist services and many community agencies.
Also helps monitor the wellbeing of those with enduring mental health needs who
are not in contact with specialist services.
